1. What records do I bring to my appointment at Well Being-Being Well?

If you are a new patient, you will need to bring any lab test and x-ray report (the report only not the actual film) from the last two years that are pertinent to the condition for which you are seeking treatment. We would also request you bring a list of all of your  prescription medications, over the counter medications , and any supplements you are currently taking. Bring this list to your first appointment and the let us know of any changes or new medications or supplements at your future appointments

2. Does Well Being-Being Well accept my insurance?

Well Being-Being Well is an out-of-network provider. We do not participate with any insurance companies. At the end of your visit we will provide you with a copy of your fee slip to submit to your insurance company or we will submit your claim electronically if that option is available. You will need to check with your insurance carrier to find out about out-of-network benefits. Please remember that your insurance coverage is based on a contract between you and your insurance carrier

The physicians at Well Being-Being Well have Opted Out of Medicare. Please note that “Opting Out” is not the same as “Non-Participating”. As a Medicare beneficiary you would pay for services at the time of service, however you cannot submit the claim to Medicare and Well Being-Being Well cannot submit the claim to Medicare. You will not receive any reimbursement for services rendered at Well Being-Being Well from Medicare. You will need to check with your secondary insurance to find out if they would pay on claims not submitted to Medicare first since many Medicare supplemental plans may not reimbursed patience for services not processed through Medicare first.

3. What form of payment does Well Being-Being Well except?

We accept cash, personal checks, visa and MasterCard. Unfortunately we do not except American Express or Discovery cards.

4. How do I make my first appointment at Well Being-Being Well?

You can download the new patient packet from the website. Please complete this packet and return it to our office either by fax or mail. Once we have received your completed packet we will make you an appointment with one of the physicians
